This form is to be used by the Members of the PTCFPA that are unable to attend the 1.25.23 Annual Members’ Meeting to register their vote for the election of the Board of Directors for 2023.

Complete this form and click SUBMIT.  Your vote MUST BE submitted by 6pm on January 24, 2023.
